Below please find a description of the use of each form.

  • Affidavit of Domicile
    A notarized affidavit executed by the legal representative of an estate reciting the residence of the decedent at the time of death.

  • Certification of Identity form
    A certification / statement verifying that you are known by two different names, yet are one in the same person. You must sign your name as both individuals and the document must be medallion guaranteed.

  • Form W-8/W-9
    A form required by financial institutions when opening up a new account. It is a certification of a taxpayer identification number (TIN). A TIN is either a Social Security number for individuals or an IRS Employer Identification number for businesses. This form must also be submitted when changing a TIN on an existing account.   • Generic Letter of Transmittal
    A document separate from the stock certificate that is utilized when surrendering shares pursuant to a company merger or tender offer.

  • Stock Power Form
    A document separate from the stock certificate that is utilized when surrendering shares to a new owner.
